The Sand-Knit tag looks to be late 60s/early 70s.

Based on logo my guess is the Denver Bears possibly, although I've never seen that style.
Always looking for Noah Syndergaard, Charlie Blackmon, Jordan Pacheco & Kenneth Faried items, thx!
