I contacted the Lookouts directly, and Rich Mozingo (Team President) said that LOA wasn't issued by them and Sally Violin was only with the team in 2003-2004. When presented with this information, the seller replied, with some of the worst grammar, stating that the LOA is what he got from the team, and he's sure the team wouldn't have forged the signatures on the item.

Apparently the owner goes by both "Terry Ellis" (per the Lookouts LOA) and "Terry Crum" (per an LOA for an overpriced McCovey ball also being sold).

I wonder if Sally Violin was fired after the 2004 season for referring to their home park as Historic Engle Engel Stadium.

Good theories, but not a woman. I've actually spoken with the seller, and he insists that the LOA is from the team, since his buddy was working for the team, but now works for a different minor league club. As for not being the first owner, that's entirely plausible.
